Effective date: 3 November 2026
At My Dental Implants, we respect your privacy and handle personal and health information in accordance with the New Zealand Privacy Act 2020 and, where applicable, the Health Information Privacy Code 2020 (including amendments effective from 1 May 2026). This policy explains how we collect, use, store, disclose and protect information when you contact us, use our website or receive dental care.
Information We Collect
We may collect your name, contact details, date of birth, appointment information, correspondence, payment and billing details, and information you submit through our website.
When providing dental services, we may collect relevant medical and dental history, medications, allergies, examination findings, X-rays, photographs, scans, treatment plans, consent records and clinical notes. We collect information reasonably necessary for lawful clinical and administrative purposes.
Our website may collect technical information such as IP address, browser details and cookie or analytics data.
How We Collect Information
We generally collect information directly from you through enquiries, forms, consultations and appointments. Where lawful and relevant to your care, we may also receive information from referring dentists, other health providers, laboratories, family members or other authorised sources.
Where required, we will explain the purposes of collection, intended recipients, whether supplying information is voluntary or required, consequences of not providing it, and your rights to access and request correction. If information is obtained indirectly, we will provide any notice required by the Health Information Privacy Code, including rule 3A, unless a lawful exception applies.
How We Use Your Information
We use information to respond to enquiries; arrange and manage appointments; assess, plan and provide dental treatment; coordinate referrals and laboratory work; maintain clinical records; issue invoices and process payments; manage safety and quality; meet legal and professional obligations; and operate and improve our services.
We use information for other purposes only where permitted by law or with appropriate authorisation. Marketing messages, where used, will comply with applicable requirements and offer an appropriate opt-out.
Sharing Your Information
We may disclose information to dentists and other healthcare professionals involved in your care, dental laboratories, diagnostic services, insurers or funders where relevant, payment processors, and contracted technology or records providers who need information to deliver services. We may also disclose information where required or authorised by law, or with your authorisation.
We do not sell patient health information. We take reasonable steps to ensure service providers protect information and use it only for authorised purposes. If information is disclosed outside New Zealand, we assess and comply with the applicable overseas disclosure requirements, including those under the Health Information Privacy Code.
Data Security and Retention
We take reasonable technical, physical and organisational steps to protect information against loss, unauthorised access, use, alteration and disclosure. Measures may include access controls, staff confidentiality obligations, secure systems and appropriate disposal processes. No online transmission or storage system can be guaranteed completely secure.
We retain personal and health records for periods required by applicable law and professional record-keeping obligations, and otherwise only as long as there is a lawful purpose. We securely dispose of information when it is no longer required.
Privacy Breaches
We assess suspected privacy breaches, take steps to contain and address them, and notify the Office of the Privacy Commissioner and affected individuals when a breach meets the statutory notification threshold, subject to applicable exceptions.
